IMC Canada is Canada's first Accountable Care Organization (ACO) specifically focused on excellence in musculoskeletal disorder (MSD) diagnosis, treatment and triage. We are aiming to establish our approach as the gold-standard system for MSD management in Canada using McKenzie MDT trained clinicians as the backbone of the organization. We are looking to build a network of MDT trained clinicians, standardized around additional training in motivational interviewing, patient engagement, outcomes data collection, and advanced MDT concepts. We track patient health data and hold all clinicians accountable for delivering the highest quality of care resulting in the best possible patient outcomes at the lowest cost. Clinicians are able to present difficult cases to our network of highly-skilled MDT practitioners across Canada and the US on a weekly basis to continuously build on their MDT knowledge and enhance clinical skills. We are fully embracing the trend in Canada towards Value-Based Health Care (VBHC). As we move from fee-for-service reimbursement to pay-for-performance or shared-saving reimbursement models, our clinicians delivering exceptional MSD care will be preferred providers for health care payers looking to reduce costs while increasing quality. We are working to establish these preferred provider networks around IMC trained clinicians across the province. We are currently launching pilot programs with Family Health Teams in London, Ontario and are looking for physiotherapists with a minimum of MDT Part A and B, or Part A and C. We are also looking for Certified MDT clinicians interested in taking advanced IMC certification training and joining our network to bring the MDT clinician community closer together. Our clinic is looking for a physiotherapist with a strong interest in treating orthopedic and sport injuries. The candidate must be a critical thinker and have a strong passion for delivering quality, evidence-based practice. This program is funded by the Central East Local Health Integration Network (Central East LHIN) and hosted by the Scarborough Health Network. It is designed to decrease the prevalence of unmanageable chronic low back pain, reduce opioid initiation, and reduce unnecessary diagnostic imaging as well as unnecessary specialist referral. The low back pain RAC is specifically for patients with: (1) persistent low back pain and/or related symptoms (e.g. sciatica, neurogenic claudication) 6 weeks to 12 months post-onset, and (2) recurrent / episodic unmanageable low back pain and/or related symptoms of less than 12 months post-recurrence. The ISAEC model of care is being implemented across the Central East LHIN as part of the MSK RAC implementation.
As a member of a RAC, the APP will act as the first and primary point of contact for consultation and assessment of referred patients, provide a defined program of care to the patient with an emphasis on education, self-management and health promotion strategies, develop a collaborative care plan with the patient and their primary care provider and document progress against the care plan, and identify signs and symptoms requiring a change in the plan of care, diagnostic imaging/testing, or referral to a Physician Specialist.
The APP is a skilled clinician, with requisite entry-to-practice training in a regulated health discipline and will be responsible for delivering rapid patient assessment and education for this specific population of low back pain patients, from their respective practice location.
The APP will be connected to a Clinical Sponsor (Physician Specialist) and Practice Leader who will provide clinical guidance and advanced low back pain education. Additionally, all APPs will be connected through a centralized referral and case management system. Please note that successful candidates must complete both practical (in-person 2 days) and theoretical training (1 week online) in low back pain topics.
